Reversal Of Parakeratosis, A Feature Of Psoriasis By Wrightia Tinctoria (In Emulsion) - Histological Evaluation Based On Mouse Tail Test

An experimental histological evaluation on reversal of parakeratosis to orthokeratosis based on mouse tail test was conducted. Wrightia tinctoria (in emulsion) when applied to mouse tail, showed nearly 90% reversal of parakeratosis to orthokeratosis, when compared with treatments using retinoids and betamethasone.

Atopic Dermatitis - A Clinical Profile

A total of 80 atopic dermatitis cases were studied. The incidence was 4.2 per 1000 among OPD patients. The overall crude M : F ratio was 1.4:1 which after standardization was 0.9:1, 32.5% had the disease for the first time, 48.75% of patients had observed itching preceding rash, 26% of patients gave history of food allergy. The incidence of personal history of atopy and family history of atopy was 56.25% and 63.75% respectively. Facial involvement was common in infant and flexural involvement in adolescent and adult. Vesiculation, papules, erythema were common in all. Lichenification was seen in chronic cases, 18% of the patients had ichtyosis vulgaris and 93% of infantile, 55% of childhood and 57% of adult cases had xerotic skin. Hyperlinear palms were seen in 38% and keratosis pilaris in 34%. Dennie-Morgan line was seen in 50% of the cases. White dermographism was demonstrated in 50% of 66 cases tested. Secondary cutaneous infections were common.

A Clinico-Microscopic Corroboration Of Surgical Repigmentation - A Study Of 30 Cases

A total of 30 cases of stable vitiligo with a positive response to punch grafting constituted the sample population. Nineteen of them were female and eleven male, age ranged between 12 and 45 years. Sixteen to twenty weeks after punch grafting, a biopsy each from normal, vitiligenous and surgically repigmented areas, were performed. Sections were studied microscopically using H&E and mason-fontana stains. The microscopic findings demonstrated a district clinico-microscopic corroboration between normal and surgically induced repigmented sites in cases of stable vitiligo.

Photochemotherapy For Widespread Psoriasis

Forty live psoriatics (40 chronic plaque, 5 stable erythrodermic) were treated with PUVA therapy. The psoriasis area and severity index (PASI) was used for evaluation. The average pretreatment PASI score was 14.6 (range 7.4-35.4) in chronic plaque psoriasis and 39.9 (22.3-52.2) in psoriatic erythroderma. Patients were given oral 8-methoxypsoralen (0.6mg/kg), thrice a week with initial UVA exposure of 3 joulrd/cm2 and increments of 1 joulrd/cm2 every week till complete clearance or up to 12 weeks whichever was earlier. After clearing, patients were put on maintenance therapy. The average response at the end of 8 weeks was 84.5% reduction in PASI score in chronic plaque and 86.5% in erythroderma. Patients received a cumulative dose of 186 and 165 joules/cm2 respectively. Based on the time required for clearance, patients could be divided into rapid responders (4 weeks, 14 cases), average responders (8 weeks, 21 cases), slow responders (12 weeks, 5 cases) and non-responders (5 cases). One patient of chronic plaque type developed pustular psoriasis. No other serious side effect was observed. Hence, systemic photochemotherapy is effective in inducing long-lasting remissions in plaque and even stable erythrodermic psoriasis. More than 70% cases were maintained in remission at 6 months, 58% at 1 year and 32% at 2 years.

Syringocystadenoma pappilliferum is a benign adnexal tumor with apocrine differentiation. It is derived from primary epithelial germ cells. It can occur as an isolated tumour or in association with naevus sebaceous of jadassohn. The common sites are scalp or face but rarely it can occur elsewhere. We present a case of syringocystadenoma palilliferum on the back.

Efficacy Of Pimozide In Delusion Of Parasitosis

Twenty patients of dulusion of parasitosis received oral pimozide (PMZ) 2 mg daily for 3 months. Trihexyphenydyl hydrochloride (THP) was administered in a dose of 4 mg daily in young adults and 2 mg daily in elderly patients for prevention of extrapyramidal side effects (EPS). Within 1-2 week of initiation of therapy, sixteen patients responded completely. In four patients, dosage of PMZ had to be increased to 4 mg daily and they responded 3-4 week after initiation of therapy. Treatment was continued for 3 months and all were symptom free. Following discontinuation of therapy, delusion reappeared but disappeared after reinstitution of PMZ therapy. EPS developed in two young patients resulting in discontinuation of pimozide.
